A 47 yr old male pedal edema,SOB

This is an online E log book to discuss our patient's de-identified health data shared after taking his/her/guardian's signed informed consent. Here we discuss our individual patient's problems through series of inputs from available global online community of experts with an aim to solve those clinical problems with collective current best evidence based inputs. History: A 47 yr old male autodriver came with the chief complaints of b/l leg swelling since 2months,shortness of breadth since and decreased urine output since 2months and Patient was apparently asymptomatic since 8yrs then he was diagnosed hypertensive on a regular checkup and 6yrs ago diagnosed as diabetic as he developed generalised weakness and wt loss and 5yrs back he was with diagnosed with acute pancreatitis and 2months back he developed pedal which eventually progressed to the kness and SOB grade 2 and then progressed grade 4 and then sob subsided he also has decreased appetite and decreased
